entered into an agreement with Southwest Community Health Center, Inc.
for deployment of NextGen®
Ambulatory EHR, NextGen®
Practice Management and NextGen®
Electronic Dental Record (EDR) across its 12 locations throughout
Southwest Connecticut.
Southwest Community Health Center (SWCHC), Inc. is a Federally Qualified
Health Center (FQHC) with more than 20,000 patients averaging more than
115,000 visits per year. Its 220 employees address various social
determinants of health by providing comprehensive, culturally competent
healthcare and social services. Primary and preventive care services
such as medical, dental, and behavioral health are provided to
individuals of all ages, regardless of their ability to pay.
Support for meaningful use and PCMH
As a Federally Qualified community health center with multiple grant
awards, SWCHC requires more than standard clinical and financial systems
to meet its robust reporting requirements. Utilizing NextGen® solutions,
SWCHC will have the integrated reporting utilities to support the
variety of documentation needed to satisfy federal and state regulations
and funding requirements, as well as disease collaboratives, auditors,
and private payers.
The Center will leverage NextGen Practice Management to increase revenue
through enhanced charge capture capabilities, elimination of document
storage fees and improved operational efficiencies. NextGen Ambulatory
EHR and NextGen EDR will integrate SWCHC's clinical, scheduling,
pharmacy and dental services to create a comprehensive medical
management platform. This in turn, supports the centers' Meaningful
Use efforts and its plans to meet Level 3 National Committee for
Quality Assurance (NCQA) Patient-Centered
Medical Home (PCMH) criteria.
SWCHC is near completion of its new 22,000-square-foot multiple-use
health center and supportive housing residence in Bridgeport, CT. This
building project is in conjunction with the Bridgeport Housing Authority
and will create 30-40 units of new affordable housing and a health
center that will allow SWCHC to house medical, dental, behavioral health
and administrative functions in one state-of-the-art facility.
